• Speak with Planes Master Jacob

This is the npc in the bazaar for the Hardcore Difficulty I missions
Choose between Plane of Hate, Plane of Fear, Plane of Growth, or Plane of Mischief mission from him

• Kill 150 Old Plane's Mobs

This is further plain and simple, it does not matter which Planes zone you are in,
or if you take on trash in all four of them, as long as you get to 150 kills.
